Hello,
I am afraid that the user accessible version of the Prime INPUT function is not yet fully featured enough to allow such a form to be created easily.
You could still create such a form, but to do so, you will have to recreate the whole thing from scratch. Doing your own draws and key handling. This is not an impossible task, but no small boat (ha ha) either.
