Maybe the flag doesn't go true unless both the head switch is closed and the buffer is ready?

It might make sense this way. For whatever reason, if the HDS was no long shorted during a card operation, then after a CRC100, s3 stays LO and would cause an error, the card would be ejected, and the process would stop.

Only conjecture.
