It seems the name was very similar to "Applied Numerical Methods" but had "with FORTRAN"
The intent of the book (in my opinion) was to give a reasonable math based somewhat "how-to" description showing the logic & pitfalls of various numerical methods specifically in a manner useful to a FORTRAN programmer.

The numerical analysis and mathematics software communities are not large...
A major motivation for writing this text was to update and expand the material in the book by G. Forsythe, et al., (1977). We liked its philosophy and informal style as well as the inclusion of programs. Our original intent was to produce a revision of that book but with Forsythe deceased..

A significant part of this book is a set of Fortran subroutines. In fact, the book might well be regarded as an extensive 'users guide' for them.
Sound familiar?
